Award Applications

To earn an award, students need to fill out the Service Award Application Summary Page and attach a Verification Form for each service activity. You may start using the 2016-2017 Verification Forms now and collect needed signatures for completed work. Be sure to keep a daily of your service hour to complete the forms and allow a verifying supervisor to sign off on your hours.
