Referral to the Wellness Center

Teachers can refer students by completing a Wellness Referral form or contacting the Wellness Center directly. 

Parents/Guardians can refer students by completing the Wellness Referral above or by contacting contacting the Wellness Center directly via telephone: (925) 280-3956 or email: chswellness@auhsdschools.org.

Students can self-refer or refer friends via the Wellness Referral form or by reaching out to Wellness staff direcly. 

If you are unsure as to whether Wellness is the best next step, please fill out a Wellness Referral. All referrals are vetted. If Wellness isn't appropriate or another resource is needed, we will make sure that the referral gets to the correct person to handle the issue.
